Obtaining a Certificate of Insurance

If you are a USS member Club or Association and need a certificate of insurance please contact Justin Deleo at Aon and copy USS Membership to the email (justin.deleo@aon.com; membership@usspeedskating.org).

• Certificate Holder: Entity Name and Full Address
• Coverages Needed: General Liability, Auto Liability, Etc?
• Description of Operations: E.g. “Waiver of Subrogation in favor of the city”
• If any additional insured’s need to be listed please provide the following information: Name (usually of the Club or Association, possibly rinks), address of Club/Association and/or rink(s) and any specific additional insured language for the request if applicable
